CRT™ PROVIDER DISCLAIMER AND TERMS OF USE AGREEMENT
As an approved CRT™ counselor we acknowledge that you have use of the proprietary software to deliver CRT to your clients. In the provision of this program, you agree to the following terms and conditions:
1. All individuals involved in the delivery of the CRT™ program must be well versed in dementia care.
2. You agree that the information you have provided to CRT™ is accurate and that you have correctly stated your professional relationship/affiliation.
3. All information obtained from the CRT™ website and software is copyrighted. Module use is priced individually, so that a module may be used for one client only. Multiple client use of the same purchased module is not permitted.
4. You acknowledge that access to client modules is dependant upon payment via PayPal for each client on the system at the beginning of each calendar month at the agreed upon monthly per client rate.
5. The author and publisher of this Program and the accompanying materials have used their best efforts in preparing this Program. The author and publisher make no representation or warranties with respect to the accuracy, applicability, fitness, or completeness of the contents of this Program. The information contained in this Program is strictly for educational purposes. Therefore, if you wish to apply ideas contained in this Program, you are taking full responsibility for your actions.
6. The author and publisher disclaim any warranties (express or implied), merchantability, or fitness for any particular purpose. The author and publisher shall in no event be held liable to any party for any direct, indirect, punitive, special, incidental or other consequential damages arising directly or indirectly from any use of this material, which is provided “as is”, and without warranties.
